This is a 70 year-old who had her initial breast augmentation (silicone, subpectoral) at age 29 with subsequent reoperations for capsular contracture of the breasts at age 50 and again at age 64. She presented with recurrent scarring and contracture and did not want further exchanges. She just wanted her implants out.
She was a D cup and was willing to be a smaller size but wanted to remain as full as possible without an implant. Other consultations had told her she would need to have Wise-pattern scars(periareolar and anchor). She had removal of her implants (385cc silicone implants on each side), skin removal from the breast fold only, and fat grafting to the subcutaneous space and around the old scar capsule (97cc left, 101cc right).
These are her two month follow up photos. She was very happy with her size and shape and is considering further fat grafting to be larger, which can be done as early as 4 months after surgery.
